A vaccine-based strategy would be particularly suited for use in the developing world, where maternal immunization with tetanus toxoid is a safe and valuable part of routine antenatal care (40). Two-thirds of African women receive some antenatal care, but only 10 percent get preventative treatment for malaria and only 1 percent of HIV-positive mothers get the treatment necessary to prevent transmission of the disease to their newborns. The development of an item number to improve access to antenatal care for women living in rural and remote areas is welcome, but asking health practitioners to provide care that is outside their scope of practice and that they are not competent to perform cannot be considered an improvement in health services. |
