The air crash was caused by the following combination of unfavorable factors: failure to comply with requirements of instructions by the crew; failure to meet requirements of the visual approach; loss of control over flight altitude in missed approach caused by incorrect assessment of the aircraft descent by the captain of the crew; failure to take actions required when the ground proximity warning system alerts by the crew.
Based on its fixed-wing enhanced ground proximity warning system, the helicopter system is modified to account for the unique rotary wing operating environment.
