In an earlier but similar study from another children's hospital in the country, Buckingham et al (2) reported that the proportion of cases of complicated parapneumonic effusions caused by S aureus increased from 6% in the period 1996 to 2000 (all of which were methicillin susceptible) to 30% in 2001 (all methicillin resistant).
The clinician must consider pulmonary embolus, viral infection, parapneumonic effusion, tuberculosis, congestive heart failure, and collagen vascular disorders--such as SLE or rheumatoid arthritis--in the differential diagnosis.
1) Pleural fluid eosinophilia most commonly arises from hemothorax or pneumothorax (2) but can also result from asbestosis, (3) pulmonary infarction, (4) parapneumonic effusion, (1,4) tuberculosis, (4) parasitic infections including toxocariasis, (5) and strongyloidiasis, (6,7) eosinophilic pneumonia, (8) malignancy, (1) and medications.
