GLENDALE - Swiftwater Rescue firefighter and paramedic Troy
Incident management, $7,000 fine: No "rapid intervention team" was in place to rescue firefighters trapped inside the building; no safety officer was assigned to supervise until after the first units arrived; no communication between incident command and the fire supervisor near the rear of the building; incident command didn't provide direction or get progress reports from crews inside the burning building; command was transferred, without everyone at the scene being informed.
According to the occupational safety report, there was no team in place to rescue firefighters that were trapped in the building.
