The EMR links the physician's notes to the SNOMED CT concept (H/O: thrombosis) or an ICD-9-CM concept (personal history of venous thrombosis and embolism).
A system like SNOMED, for example, cannot transfer data into a system like ICD-9-CM without losing specificity.
