Although CVID patients with TNFRSF13B mutations show normal total peripheral B-cell numbers, these patients have a diminished CD27-positive memory B-cell population, with a severe decrease in the switched memory B-cell compartment (CD27+, immunoglobulin D [IgD]-) B-cell population (Figure 1).

Underlying immunodeficiency can cause sudden onset pulmonary involvement and immunologic reactions to fungal or bacterial agents such as common variable immunodeficiency (CVID), chronic granulomatous disease (CGD), hyper-IgE syndrome, and human immunodeficiency virus (HIV) [19, 20]; yet, in our cases immune work ups for antibody and cellular primary and secondary immunodeficiency were negative and there was no sign of immunodeficiency in her history and physical exam.
Various disorders within this category have been identified, including X-linked agammaglobulinemia (XLA), common variable immunodeficiency (CVID), IgA deficiency, and autosomal recessive agammaglobulinemia (ARA).
