In most cases, PTA and its variants have been found incidentally but can be associated with numerous anomalies in the cerebral vasculature, including arteriovenous malformation, aneurysms of the circle of Willis, carotid and vertebral artery agenesis, carotidcavernous fistula, and moyamoya disease [3, 7].
The axial T2-weighted MR images (Figures: 2b, c) and source images of 3D TOF MRA (Figures: 2d, e) show collaterals around circle of Willis, in the ambient cisterns and sylvian valleys.
On imaging with four-vessel angiography and computed brain tomography, a bullet was noted in the right internal carotid artery with no distal flow, the circle of Willis was patent, and a right fronto-parietal infarct was seen on the brain scan (Figs 1 and 2).
