Microarray analysis of allergic fungal sinusitis and eosinophilic mucin rhinosinusitis.
Noninvasive disease comes in two forms: Allergic fungal sinusitis and mycetoma ("fungus ball").
The syndrome has been reported in patients using humidification reservoirs for C-Pap therapy, and it can coexist with allergic fungal sinusitis. It is also more common in humid, coastal areas.
However the question arises whether fungi play a role in the persistence of polypi, recurrences of polypi and whether they contribute to the persistence of inflammation and edema as in allergic fungal sinusitis (AFRS).
In 1989, Robson et al (6) proposed the term allergic fungal sinusitis, because they identified dematiaceous fungi instead of Aspergillus species in their patient.
ALLERGIC FUNGAL SINUSITIS (AFS): Allergic fungal sinusitis occurs when a fungus colonizes a sinus cavity and then causes allergic mucosal inflammation through an IgE response to fungal protein.
Since that time, numerous other fungi, including Curvularia, have been identified to cause the same clinical entity, and the term allergic fungal sinusitis (AFS) was coined.
Approximately 10% of patients with chronic rhinosinusitis or nasal polyposis have concurrent allergic fungal sinusitis. This rate is higher in patients with asthma, allergies (atopic), and allergic bronchopulmonary aspergillosis.
[1] T Robson et al [2] in 1989 introduced the term Allergic fungal sinusitis (or allergic fungal rhino sinusitis) to describe a constellation of unusual findings in a unique group of patients suffering from chronic sinusitis.
The patient did have allergic fungal sinusitis and presented as you described, with typical allergic mucin.
Allergic fungal sinusitis (AFS) was first described in the literature in the early 1980s, when Millar et al noticed a clinical entity of sinus disease that was similar in many ways to allergic broncho pulmonary aspergillosis (ABPA).
The two most common concomitant diagnoses in the study population were allergic fungal sinusitis (13.5% of patients) and cystic fibrosis (8.9%).
