(4-6) In a study measuring the effectiveness of anti-reflux surgery, Oh et al (6) showed a significant decrease in postoperative hospital visitation scores associated with reflux proving that anti-reflux surgery has significant benefits for NI children.
When a patient with an oesophageal GERD syndrome is responsive to, but intolerant of, acid suppressive therapy, who want to discontinue maintenance treatment or when the reflux of stomach contents causes troublesome symptoms and/or complications, anti-reflux surgery should be recommended as an alternative if adequate and safe surgical expertise is available (A 1b).
During the study period, total of 7000 patients underwent one of four procedures: appendicectomy (n = 3520), cholecystectomy (n = 2250), anti-reflux surgery (n = 720) or gastric bypass (n = 1510).
The recurrence of GER symptoms has been studied in regard to long-term outcomes of anti-reflux surgery. Rates of recurrence of GER following TF range from 1% to 25% (Goessler et al., 2007; Montupet, 2002), with higher rates again associated with neurologically impaired patients.
Although most people find relief through medications, recent research suggests that anti-reflux surgery may offer good long-term results.
But for people with chronic GERD, another option is anti-reflux surgery. This laparoscopic surgery involves reinforcing the malfunctioning valve between the esophagus and the stomach to stop reflux.
Fundoplication patients showed higher LES resting pressures (35 mmHg vs 23.5 mmHg, P < 0.01) compared with patients without previous anti-reflux surgery. There was no association between esophageal motor disturbances and MECP2 mutations.
In some cases, such as cholecystectomy or anti-reflux surgery, laparoscopy has become the unquestioned standard of care; in others, such as hernia or colorectal surgery the debate continues.
These data, in conjunction with previous studies that indicate that anti-reflux surgery produces dramatic relief of respiratory symptoms (Kennedy 1962; Overholt and Voorhees 1966; Davis and Fiuzat 1967; Skinner and Belsey 1967; Urschel and Paulson 1967; Babb and others 1970; Lomasney 1977; Henderson and Woolfe 1978; Pellegrini and others 1979), support a possible role for non-acid reflux in the persistence of respiratory symptoms following acid suppression therapy in children with acid GER.
It may also be that GERD contributes to asthma in some way, since at least one study finds that anti-reflux surgery can significantly improve asthma symptoms in patients with both.
Their findings, when compared to the requirements of performing anti-reflux surgery, clearly demonstrate the Stretta procedure's cost effectiveness for patients who would be candidates for either procedure."
