The aim of our CNIR service is to provide a community alternative to hospitalisation for patients who need specialised nursing care on discharge.
The CNIR service helps to reduce prolonged in-patient stays and hospital admissions, in turn reducing pressure on acute beds.
The CNIR nurse meets the patient in the hospital prior to their discharge, so that any concerns and anxieties may be identified and actions taken to address them.
The CNIR nurse is also able to provide on-going teaching and support for colorectal patients in order to encourage independent living.
By assessing patients with specialist care needs within the community, the CNIR nurse can avoid unnecessary attendance at hospital.
