Contributors from physical therapy, oncology, rehabilitation, and other areas that deal with lymphedema set out the core principles of manual lymph drainage and complete decongestive therapy for a wide range of pathologies across the age continuum.
Recently, standard protocol for the management of LE includes complete decongestive therapy (CDT), which comprises the use of manual lymph drainage (MLD), daily bandaging, skin care, exercise, and compression.
Complete decongestive therapy was well-tolerated by the patients and no adverse effect or worsening in LE was reported.
Some authors have advocated that complete decongestive therapy significantly decrease girth together with Visual Analog Scale scores for pain.
1] The guidelines recommend complete decongestive therapy (CDT), which includes gentle massaging known as manual lymph drainage, use of a multilayered compression bandage to the affected limb, exercise, and skin/nail care.
It has more on bandaging and taping techniques for the upper and lower extremities, compression garment selection and care, genital manual lymphatic drainage and bandaging, and complete decongestive therapy adaptations for various complications, as well as updates to sections on patient evaluation, early detection/surveillance, exercise, risk reduction, diagnosis, lymphatic filariasis, axillary web syndrome, Klippel-Trenaunay and Parkes Weber syndromes, and wound care.
Early chapters cover the anatomy, physiology, and pathology of the lymphatic system, and later chapters explain manual lymph drainage (the Vodder technique), complete decongestive therapy, and other treatment modalities.
Complete decongestive therapy (CDT) is the recommended treatment for lymphedema and is performed in two stages.
Interventions for Lymphedema Recommended for Practice * Complete decongestive therapy * Compression bandaging * Infection treatment Likely to Be Effective * Maintaining optimal body weight * Manual lymph drainage Benefits Balanced With Harms * Exercise * Prophylactic antibiotics for recurrent infections * Surgical intervention Effectiveness Not Established * Compression garments * Hyperbaric oxygen * Low-level laser therapy * Nanocrystalline silver dressing on lymphatic ulcers * Pneumatic compression pump * Simple lymphatic drainage with aromatherapy Not Recommended for Practice * Diuretics * Benzopyrenes Note.
Because the difference of diameter does not require the compression bandage step of the complete decongestive therapy (CDT), its steps outside bandage (skin care, self-massage, compression, exercise, and diet) were administered.
