Our primary objectives were to assess the placental disposition of methadone and EDDP in opioid-dependent pregnant women and to explore the correlations with maternal methadone dose, placental methadone concentration, and neonatal outcome.
Placenta samples were stored at -20 [degrees]C until analysis for methadone, EDDP, morphine, codeine, 6-acetylmorphine, cocaine, and benzoylecgonine (BE) by a validated liquid chromatography-ion trap mass spectrometry method (7).
The Kolmogorov-Smirnov test evaluated the fit of data to a normal distribution, and Pearson correlation analysis assessed relationships among placental methadone and EDDP concentrations, EDDP/methadone concentration ratios, maternal methadone dose, and neonatal outcomes (except for 5-min Apgar scores, for which Spearman correlation analysis was applied because of data nonnormality).
Placenta samples from 3 additional participants were collected and used to test methadone and EDDP distribution across the placenta; however, these samples were not included in further data analysis because maternal methadone-dosing and neonatal-outcome data were not available for these participants.
DISTRIBUTION OF METHADONE AND EDDP ACROSS THE PLACENTA
Methadone and EDDP may well be absent in the urine initially, but will appear when the dosage has been increased to a sufficient amount.
The method would also confirm and quantitate EDDP, if present.
These samples were negative for methadone at the limit of detection of the assay; in addition, no EDDP was detected.
The largest differences in identification rates by country corresponded (in decreasing order) to 6-acetylmorphine, EDDP, benzoylecgonine, ecgonine methyl ester, and 11-nor-9-carboxy-[[DELTA].sup.9]- tetrahydrocannabinol.
Discrepancies in the mode were found for the following substances: amphetamine, 300 [micro]g/L for clinical laboratories vs 200 [micro]g/L for forensic laboratories; 6-acetylmorphine, 300 vs 10-50 [micro]g/L; EDDP, 300 vs 200 [micro]g/L; ecgonine methyl ester, 20 vs 100 [micro]g/L.
