Performance measures for this project included the number of patients (a) with COPD discharged from the hospital, (b) who participated and those who declined to participate, (c) in each grade of dyspnea on the MRCDS, (d) who followed APRN recommendations, (e) with a dyspnea Grade 0 or 1 who were readmitted in less than 30 days, and (f) with a dyspnea Grade 0 or 1 who adhered to routine follow up.
All 52 project participants received a telephone assessment within 5 days of hospital discharge using the MRCDS to evaluate the level of dyspnea (see Figure 1).
Further MRCDS during the hospital discharge process can establish an individualized benchmark for comparison to the follow-up telephone assessment.
