We tested for cost shifting after the start of the mental health carve-out using two types of models run for each of the four major types of expenditures: inpatient psychiatric care, outpatient psychiatric care, non-psychiatric medical care, and pharmacy.
For children, there were modest increases in expenditures on outpatient psychiatric care and pharmacy, and a similar, but statistically insignificant, decrease in expenditures on inpatient psychiatric care.
As practice patterns change and more outpatient psychiatric care falls into the purview of the family physician, the two physicians see a need for ever more collaboration between their specialties.
Other changes in the health care plan (most of which are effective January 1, 1991) include the requirement for a second surgical opinion for certain surgical procedures; a newly established copayment of $3 for generic drugs and $7 for brandname drugs; preventive mammography screening; payment of outpatient psychiatric care at 60 percent of reasonable and customary costs; payment at 80 percent for certain other outpatient sevices, such as pre-admission testing and second surgical opinions; improvements in dental coverage; and various changes to post-retirement health care benefits.
Over the next 6 years, the legislation will phase out higher co-payments for mental health services under Medicare Part B from the current 50% for outpatient psychiatric care to 20% in 2014.
The recommendations are designed to address two undeniable truths about the transition from acute to outpatient psychiatric care: Most patients who are referred for outpatient psychiatric care upon discharge do not get it, and the law, with few exceptions, can't force them to so.
Another reality of reimbursement--cutbacks in private sector insurance mental health benefits--supports a cash-practice model for outpatient psychiatric care from the patient's perspective.
