The present case was successfully treated with surgical removal of HO and the peroneus quartus muscle, together with peroneus longus transfer to the peroneus brevis tendon.

It passes deep to the peroneus longus and continues anteroinferiorly between the peroneus longus, peroneus brevis, and extensor digitorum longus before piercing the deep fascia in the distal third of the leg.
