SHML has been found to occur concomitantly with other diseases such as diffuse large B-cell lymphoma, [1] HIV infection, [2] varicella zoster infection and autoimmune lymphoproliferative syndrome.
Uniquely, although extranodal sites of SHML are typically found in the head and neck, they rarely involve the salivary glands (1).
Histiocytes in SHML characteristically exhibit emperipolesis: a process by which intact cells are found in the cytoplasm of the histiocytes.
Previous reports of SHML with extranodal manifestation in the parotid gland refer to patients with an established immunologic disease (5), and it has been noted that patients with extranodal involvement of the head and neck tend to have higher rates of immunologic abnormalities (6).
There are only 20 reported cases of SHML identified in association with both non-Hodgkin lymphoma and Hodgkin lymphoma, usually involving different anatomic sites and developing at different times.
The SHML was present in multiple areas throughout the lymph node and comprised approximately 40% of the lymph node.
The distinctive histiocytes of SHML with emperipolesis are different from histiocytes seen in a variety of other benign and malignant disorders--such as Hodgkin's lymphoma, monocytic leukemia, rhinoscleroma, juvenile xanthogranuloma, and Langerhans histiocytosis--in which phagocytosis of cells may be a prominent feature.
The pathogenesis of SHML has not yet been established.
In 1988, the first two cases of SHML were known with epies-cleral mass and cervical and inguinal lymphadenopathy in patients less than 20 years.
3) In 1972, Rosai and Dorfman analyzed 30 additional cases, establishing SHML as a well-defined clinicopathologic entity.
Approximately 600 cases of SHML have been described in the English literature, generally children or young adults with tumoral cervical adenopathy, fever, hypergammaglobulinemia, an increased erythrocyte sedimentation rate (ESR), and leukocytosis with neutrophilia.
