Lynn et al., "The effect of temporal artery biopsy on the treatment of temporal arteritis," Am J Surg, vol.
Temporal artery biopsy showing mononuclear cell infiltration (H-Ex200).
Temporal artery biopsies were common in all cases tested, (4,8,11,14) except in the case reported by Horne et al, (10) in which the temporal artery biopsy done at the time of the mastectomy showed arteritis.

De Jesus-Monge noted that the diagnostic clinical criteria for giant cell arteritis include being over age 50 years and having a new onset of headache, an abnormal temporary artery on physical exam, an ESR by Westergren over 50 mm/hr, and a diagnostic temporal artery biopsy. The finding of at least three of these five criteria has a sensitivity of 93.5% and a specificity of 91.5% for the diagnosis.
Temporal artery biopsy revealing panarteritis with dense mixed inflammatory infiltrate, disrupting external and internal elastic lamina (H-E x 4 and H-E x 10).
The patient's course between 2007 and the most recent study in 2016 included an extensive rheumatologic and hematologic workup, including a temporal artery biopsy in 2015.
The diagnosis of GCA is made by a number of criteria including an elevated erythrocyte sedimentation rate (ESR), C reactive protein (CRP) and positive temporal artery biopsy. (7)
These cohort studies are subject to verification bias, as most cohorts represent a selected sample of patients who had a positive temporal artery biopsy. The authors of the 2005 decision analysis note that unilateral temporal artery biopsy has a mean sensitivity of 86.9% (95% confidence interval [CI], 83.1%-90.6%) when compared with a gold standard derived from bilateral artery biopsy, American College of Rheumatology criteria, or clinical diagnosis.
Temporal artery biopsy showing granuloma infiltration of the vessel lumen will confirm the diagnosis.
Temporal artery biopsy revealed classic temporal arteritis including giant cells and fragmentation of the elastic lamina (figure).
The diagnosis can sometimes be confirmed by biopsy, but temporal artery biopsy is usually negative.
