USMT encompass a large group of gynecological tumors ranging from those with favorable outcome (LM), to the highly aggressive ULMS, and in between these 2 extremes are tumors with borderline or intermediate behavior called STUMP.
Subtyping of USMT is very significant due to the great diversity of clinical behavior and patient morbidity and mortality among those tumors.
The malignant behavior of USMT had been determined by a set of histopathological features called Stanford criteria (Bell et al.) that include: diffuse moderate-to severe nuclear atypia; mitotic count of [greater than or equal to] 10 /10 High Power Fields; and Tumor Coagulative Necrosis.
In addition to histopathological findings, the presence of diffuse p16 expression and p53 overexpression can be used as evidence to distinguish between benign and malignant USMT. Close monitoring of p53-negative LMS cases should be recommended to detect metastasis.
Objective: This study aimed to evaluate diagnostic parameters such as mitotic count and tumor size in; leiomyosarcomas (LMSs) and benign uterine smooth muscle tumors (USMTs) and to define the diagnostic value and prognostic significance of the Ki-67 proliferation index and p16, p53, and bcl-2 expressions by immunohistochemical (IHC) methods.
Conclusion: In addition to histopathological findings (tumor size and mean mitotic count), diffuse p16 expression and p53 overexpression can be used to distinguish between benign and malignant USMTs. A high mitotic index [[greater than or equal to]10/10 (high-power field)] and high Ki-67 expression (>10%) can serve as useful indicators for diagnosing LMS, distinguishing benign tumors, and predicting an aggressive clinical course.
Another commonly used hormone therapy for leiomyoma is a gonadotropin-releasing hormone (GnRH) agonist, which down-regulates GnRH receptors in the pituitary gland, decreases the level of luteinizing and folliclestimulation hormones, and thus results in the required hypoestrogenic state that will help reduce USMT size.
Uterine smooth muscle tumors (USMTs) that cannot be categorized confidently as benign or malignant continue to be common diagnostic dilemmas in everyday practice.
